Coaches Paperwork (over 18 by Aug 1)
Coaches need to fill out the following forms:

• BYFA Coaches Application
• NHYFSC Volunteer Application
• NH Background Check form**

**This form needs to be notarized.

Student Demonstrators/Coaches in Training (ages 15->17)
Student Demonstrators and Coaches in Training need to fill out a large amount of paperwork. Basically what we need for each Sd/CIT is the same paperwork we need for each cheerleader, and what we need for each coach.

• Please Start by going to the State web site and Pre-register
• We need the top of this form filled in and signed by a parent
• This form needs to be signed by a parent and by you.
• BYFA Coaches Application
• NHYFSC Volunteer Application
• We need a copy of your birth certificate if you were not a student demonstrator in 2007.
• We need a note from your doctor, from a 2007 or 2008 physical, that is signed in 2008, that says you are allowed to participate in sports.
• We need an end of the 2007 year report card.
